NEW PATIENT REGISTRATION

**Our practice list is currently closed for NEW patient registrations**

Note: We are still accepting newborn babies to existing patients. We are still accepting relatives of existing patients - Proof of address MUST be provided in order for this to be accepted. 

Please complete our practice registration form. Once completed, please return to a member of our Reception Team. In order for your registration form to be accepted by the practice, we will require two copies of ID - 1)  proof of address (i.e. - bank statement / utility bill) ensuring you fall within the practice catchment area. 2) Photographic ID (i.e. - Passport). Once required documentation has been gathered, we will then start the process of adding you into our practice list - THIS CAN TAKE UP TO TWO WEEKS, SO PLEASE ALLOW TIME FOR THIS PROCESS TO BE CARRIED OUT. 

THE PRACTICE AREA 

Our catchment area is presently under review. Due to the demands on our clinical services, we have had no choice but to request patients out with our boundary, to register with GP surgeries nearer to their home address. Our existing boundary is likely to be reduced again in the very near future. 

CHANGES TO PERSONAL DETAILS

Please advise our Admin Team should you change your name, address, telephone number or any other personal information that we may wish to record in your medical records.  Remember to also inform any support services (i.e. the hospital, physiotherapy service etc.) you are attending or are awaiting to attend in the future.   Please note, if you move house out with our practice catchment area, you should register with a new GP as soon as possible, as we are unable to provide medical services out with our practice boundary.
